Prior to representing a client, Kearney & Phelan requires each potential client to execute an individual representation agreement for each individual property. Once a representation agreement has been executed, clients must then execute the following documents required by the Cook County Assessor's Office and the Cook County Board of Review.

Cook County Assessor Owner Lessee Verification Form
The Cook County Assessor Form must be printed and signed by one of the property owners, agents or member of the Condominium Association. Forms can be faxed to Kearney & Phelan, Ltd at 312-263-6465.

Owner/Lessee Attorney Verification Form for the Cook County Board of Review
This form requires the client's ORIGINAL signature. Please complete this form and MAIL the original form to Kearney & Phelan, Ltd. at the address listed below. If the 2012 Form is not available on our site, please use the form provided and edit the Tax Year Appeal by hand, replacing 2011 with 2012. We will provide the 2012 Form when released from the Board of Review.
