Prior to representing a client, Kearney & Phelan requires each potential client to execute an individual representation agreement for each individual property. Such agreements are not provided on this website and must be obtained through consultation with an attorney. Please call 312-263-6456 to schedule a free consultation.
Once a representation agreement has been executed, clients must then execute the following documents required by the Cook County Assessor's Office and the Cook County Board of Review.

Owner / Lessee Verification Affidavit for the Assessor's Office
In order for Kearney & Phelan to represent client's properties in the Cook County Assessor's Office, clients must complete and fax this form to our office at 312-263-6465.

Owner/Lessee Attorney Verification Form for the Cook County Board of Review
This form requires the client's ORIGINAL signature. Please complete this form and MAIL the original form to Kearney & Phelan, Ltd. at the address listed below. If the 2011 Form is not available on our site, please use the form provided and edit the Tax Year Appeal by hand, replacing 2010 with 2011. We will provide the 2011 Form when released from the Board of Review.
